WRHA Healthy Aging Resource Team (HART)
The Healthy Aging Resource Team is a group of community health care professionals that are dedicated to helping those 55 years of age and over to live well. Our team consists of a nurse and two dietitians who work together to provide a wide variety of health services and community supports. These services (a Winnipeg Regional Health Authority program) are covered under Manitoba Health.
What can we do for you?
Connecting You to Health Resources
We help people connect with supports and resources in your community. Are you trying to access a community health resource but are not sure where to start? Are you not sure what resources are available to you? Give us a call and we can help you navigate the system.
We are available for one on one education sessions. Have you been recently diagnosed or are living with a chronic disease (ex. diabetes, high blood pressure, or cholesterol)? Give us a call and we can work with you to help you manage your chronic illness.
Community Health Presentations
We are available for community health presentations to small or large groups of people. Are you interested in learning about certain health topics? The team uses a variety of educational presentations, games and interactive displays to help you learn how to care for your health.
