The Healthy Aging Resource Team (HART) is a community-based program within the Winnipeg Regional Health Authority (WRHA). Their goal is to help those 55 years of age and over to live well. The St. James/Assiniboia/Assiniboine South Team is made of 3 facilitators – Taylor McMillan, Amy Krahn and Maria Sytnick. Each Facilitator is a registered health care professional. The team works 1:1 with individuals, families, community groups and agencies, as well as other programs in the Winnipeg Regional Health Authority (WRHA). They provide a wide range of health services and community supports through client assessments and consultations in person or by phone.
